Job Description

Speaking with patients suffering from Hepatitis-C &B and counseling them for health outcomes management.
One to one counseling of patients under multiple healthcare practitioner
Handling patient queries and providing a response from pre-defined FAQs
Guide the patient regarding the availability of drug through service centers/hospital pharmacy etc. as per directions of doctor/hospital management.
Enroll new patients into PRAYAS program and explain the various benefits of the program.
Collate health records related data from consenting patients as per direction from the doctor/protocol of hospital/Institute.
Enter data into a patient data base as per requirement/protocol of hospital/Institute and communicate the same to PRAYAS.
Receive regular feedback from the patient on compliance of therapy, periodic diagnostic testing and facilitate the next due appointment dates of the patient in consultation with the doctor.
Observe proper chain of command by reporting to the doctor and/or designated staff member (e.g., Assistant doctor or Supervisory Counselor) and to follow their directions and instructions.
Coordinate with local sales team regarding availability of drug/ any other specific request from the doctor/hospital Example- Screening camps (activation manager)
Identifying high prevalence areas of HEP C ins specified locations
Arranging Hepatitis camps as per doctors instructions and in coordination with activation manager
Sharing patient data with PRAYAS team for enrolling patient on the program (patient details/PRAYAS coupon/doctor prescription for diagnostic test and patient status i.e. regularity of therapy)
Informing patients about report status post report generation as per confirmation from PRAYAS team.
Maintain professional relationships with patients based on mutual trust and respect and in accordance with the hospital policies/guidelines.
