Job Description
- Educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing.
- Responsible to improve the patient satisfaction (NPS) Score to ensure quality service experience at Clinics
- Collects patient data to monitor health status and proactively manage the patient's condition. Typically required to be a Certified Diabetes Educator (CDE).
- Typically reports to a manager or head of a unit/department.
- Promote the mission, vision, and values of the organization
- Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection
- Assist patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and noninsulin dependent diabetics
- Assess, monitor and implement treatment plans for patients at various stages of the disease.
- The focus is on prevention of complications and management of stable diabetes
