Job Description
1. Adhere to the nursing care plan defined by a team of intensivists and nursing lead.
2. Monitor, perform/administer and record all duties/nursing care as per pre-defined nursing protocols.
3. Providing timely and accurate information to nursing managers, family and intensive care doctors on the patients' daily condition.
4. Events, if any, have to be recorded and handed over accurately during shift handovers to the next duty nurse or doctors on visits.
5. Performing procedures which will include but not limited to
I ) Systemic Procedures
a. Tracheostomy Care and Management - suction , dressing of the wound etc
b. Maintaining Saturations - providing O2 support as per the requirement and ensuring humidification is adequate. Check for patient discomfort if any
c. Invasive Catheters/Drains Care & Management - i) Central Line Care and Management - administering medications through the CVC dressing of the CVC ii) PICC iii) Urinary Catheters etc iv) IC Drains - dressing and care
d. Nutrition -
i) Maintaining nutrition via Oral, NG, PEG. Nurse Practitioners should be extremely confident in feed administrations, care and contraindications if any. Should be hands-on in managing patients with feeds via a feeding pump
ii) Should be able to manage TPN / PPN - should have understanding and confidence to handle patients on TPN/PPN Administration via PICC or IV
e. Drugs Administration - extremely conversant and hand-on with drug dosage calculations, should be well versed with all routes of drug administration. ( Oral, Parenteral IV, IM, SC, Insulin Administrations etc )
f. Input - Output Management - Should understand the intake output charts, their significance and should be able to highlight alterations/abnormalities if any.
g. Integumentary System Care and Management - Pressure Ulcers Prevention and Management - should be well versed with advanced dressings for the same and should have great emphasis and understanding of the importance of their prevention
h. Stoma Care and Management - should be hands on in colostomy, urostomy and ileostomy care along with the dressings and management. Should be able to even attend to complications if any.
i. BLS Care - Should be adept in understanding EWS and prepare for BLS in the event of unresponsiveness due to cardiac or resp arrest.
j. Patient / Family Education - Keeping a good and relevant communication flow between themselves and family by updating them on the condition of their patient, supporting them emotionally as and when needed. Should be able to handle irate questions, cross-questions and be able to call for help at the right time.
k. Emergency Nursing Care - should be a go-getter and should be in a position to identify an emergency, perform under stressful situations, take orders on phone from doc if needed, should be confident in administering emergency drugs - should be confident to be able to revive the patient, stabilize and then mobilise to agreed facility
l. In the event of an unforeseen outcome should not panic, should be trained to mediate for necessary things - eg speaking to the family, coordinating with NHHS for other support required etc.
Required but not Mandatory :
a. Chemo port Care and Administration of fluids/drugs through the port
STOCK MANAGEMENT
1. Managing drugs, consumables, surgical accurately and on a timely basis.
CLINICAL DOCUMENTATION
1. Maintain Nursing notes and reports on a real-time basis
LIAISE FOR DIAGNOSTICS, PHYSIOTHERAPY / SPEECH THERAPY / DOCTOR CONSULTATIONS WITH TEAM MEMBERS
