Job Description:
- Review medical reports and ensure accurate, timely and appropriate assignment of ICD-10-CM (diagnostic) and CPT (procedural) codes with appropriate modifiers, supply codes, PQRS, ICD-10 PCS, etc. wherever applicable to E/M/ Surgery/ Ancillary charts in accordance with coding rules and regulations.
- Assigns and sequence codes based on medical record documentation.
- Abstracts and enters coded data and designated quality management data for statistical and reporting requirements.
- Communicates documentation improvement opportunities and coding issues (discrepancies, physician queries, etc.) to the appropriate personnel for follow up and resolution.
- Maintenance of daily and monthly production details.
- Quality checking of the coding of the documents done by the associates.
- Responsible for maintaining current knowledge of coding guidelines and relevant federal regulations.
- Undertakes denial follow-up and appeals work wherever required.
- Serves as a functional resource for entry-level coders and mentors/trains other coders as needed.
- Codes any and all types of E/M/Surgery/Ancillary patient records (E/M -Office visits, Inpatient visits/Admission and Discharge, Consultations; Surgery: Surgeries in Oncology from any body part/anatomical area; Ancillary: Injections/Infusions, Chemotherapy, Specific tests in Oncology, Radiation Oncology/Treatment Planning)
Educational Qualification:
- Graduation preferably in Medical Paramedical Courses: BHMS, BAMS, BSc. Nursing, BPT, BSc. Ophthalmology/Radiology, Speech and Hearing, Bsc. Microbiology/Biotechnology, etc. with indepth knowledge in Anatomy and Physiology, Medical Terminology and hospital functions.
- Good analytical and communication skills and enthusiastic in learning and researching
