Job Description
Job Summary:
A Home Care Nurse is a licensed healthcare professional who provides nursing care to patients in their homes. This position requires the delivery of skilled, compassionate care to individuals recovering from illness, injury, or managing chronic conditions, as well as assisting in activities of daily living. The Home Care Nurse works in collaboration with physicians, caregivers, and family members to implement personalized care plans and promote the patient’s health and well-being.
The ideal candidate will have strong clinical skills, excellent communication, and a deep sense of compassion and empathy for patients. This position involves traveling to patients' homes, so a high level of independence and responsibility is required.
Key Responsibilities:
1. Patient Assessment and Care Planning

Initial Assessment: Conduct a comprehensive assessment of the patient’s health, physical condition, emotional status, and medical history in the home setting.

Care Plan Development: Work with the Home Care Manager, physicians, and other healthcare professionals to develop individualized care plans based on patient needs.

Monitoring Health Progress: Regularly evaluate patient conditions to determine the effectiveness of the care plan and adjust it as needed.

Chronic Condition Management: Provide education and support to patients managing chronic conditions such as diabetes, heart disease, or respiratory issues.
2. Clinical Nursing Care

Medication Management: Administer prescribed medications and monitor for side effects or adverse reactions. Educate patients and caregivers on proper medication usage.

Wound Care: Provide wound dressing and management for patients with injuries, post-surgical recovery, or pressure ulcers.

Intravenous (IV) Therapy: If qualified, administer IV medications or fluids as prescribed, including monitoring for complications.

Vital Signs Monitoring: Regularly monitor and document vital signs such as blood pressure, temperature, pulse, and respiration.

Assist with Mobility: Help patients with mobility and positioning, including using assistive devices like walkers, crutches, or wheelchairs.

Post-operative Care: Provide care for patients recovering from surgery, assisting with physical therapy exercises, and managing post-operative pain.
3. Patient and Family Education

Health Education: Educate patients and families about managing their medical conditions, symptoms, medication regimens, and self-care practices.

Chronic Disease Education: Offer guidance on lifestyle changes to manage chronic conditions (diet, exercise, stress management, etc.).

Caregiver Training: Provide training to family members or other caregivers on proper patient care techniques, including mobility assistance, bathing, and feeding.
4. Documentation and Reporting

Accurate Records: Maintain detailed and accurate patient records, documenting all assessments, care interventions, treatments, and patient responses.

Care Plan Updates: Regularly update care plans based on patient progress or changes in health status.

Communication: Report changes in patient conditions to the Home Care Manager, physicians, and family members as needed. Participate in team meetings to discuss patient care.
5. Coordination of Care

Collaboration with Healthcare Team: Work closely with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic patient care.

Family Support: Communicate regularly with family members regarding patient progress, concerns, and changes in the care plan.
6. Infection Control and Safety

Infection Control: Adhere to all infection control protocols to protect both patients and the nurse from potential infections, especially in vulnerable individuals.

Patient Safety: Identify safety hazards in the home environment and educate the patient and family about fall prevention and other safety precautions.
