Position Overview:
A Home Care Nurse provides personalized medical care to patients in the comfort of their homes. This role involves assessing patient needs, administering treatments, monitoring health conditions, and offering emotional support to patients and their families. The position is ideal for a caring, compassionate, and skilled nursing professional who values patient-centered care and wishes to make a difference in a home environment.
Key Responsibilities:
1. Patient Care & Monitoring

Assess the patient’s health condition and create a suitable care plan in consultation with doctors.

Monitor vital signs such as blood pressure, pulse, temperature, and oxygen levels.

Track progress and report any changes in the patient’s health to the supervising doctor or healthcare team.
2. Medical Procedures & Treatment
-
Administer medications and injections as prescribed.

Perform wound care, dressing changes, and post-surgical care.

Assist with medical equipment such as oxygen machines, catheters, feeding tubes, and IV drips.
3. Rehabilitation & Therapy Support
-
Assist patients with mobility exercises and basic physiotherapy under guidance.

Educate patients and families about health management, diet, and recovery routines.

Support recovery plans for post-hospitalization patients.
4. Personal Care Assistance
-
Help patients with daily living activities such as bathing, grooming, feeding, and toileting if required.

Ensure patient comfort and hygiene are maintained.
5. Documentation & Communication

Maintain accurate patient records including treatments provided, vitals, and observations.

Communicate with family members about patient health, care instructions, and necessary precautions.

Coordinate with doctors, pharmacists, and other healthcare providers as needed.
Requirements:

Diploma / Degree in Nursing (GNM, B.Sc Nursing, or equivalent).

Valid nursing license/registration as per state or national requirements.

Minimum 1 years of nursing experience (home care experience preferred).

Knowledge of basic medical procedures, medications, and emergency care.
