Job Description

Job Summary:

We are seeking a compassionate and skilled Home Care Nurse to provide high-quality medical and personal care to patients in their homes. The Home Care Nurse will be responsible for assessing patient health, developing and implementing individualized care plans, administering medications, and collaborating with physicians and other healthcare professionals. This role requires strong clinical knowledge, excellent communication skills, and the ability to work independently in a variety of home settings. The ideal candidate is empathetic, attentive, and dedicated to enhancing the quality of life for patients and their families.

Key Responsibilities:

Patient Assessment and Care Planning:
- Conduct comprehensive assessments of patients' physical and emotional health.
- Evaluate home environments to ensure they are safe and conducive to recovery or daily living needs.
- Develop and implement individualized care plans based on the physician’s orders and patient needs.
- Monitor and document changes in patients' conditions and adjust care plans as necessary.
Clinical Care and Support:
- Administer medications, injections, wound care, IV therapy, and other treatments as prescribed.
- Assist with daily living activities such as bathing, dressing, grooming, mobility, and toileting.
- Operate and monitor medical equipment, such as oxygen machines, feeding tubes, and blood pressure monitors.
- Provide post-operative care and monitor recovery progress following hospital discharge.
Health Education and Counseling:
- Educate patients and family members about diagnoses, medications, diet, exercise, and care techniques.
- Offer guidance on disease prevention, rehabilitation, and maintaining a healthy lifestyle at home.
- Train family members or caregivers on how to perform certain medical tasks safely.
Coordination of Care:
- Collaborate with physicians, physical therapists, social workers, and other healthcare providers.
- Coordinate follow-up visits, referrals, and services such as laboratory tests or equipment delivery.
- Communicate effectively with family members to ensure seamless, coordinated care.
- Maintain up-to-date records and ensure compliance with healthcare regulations and protocols.
Emotional and Social Support:
- Provide emotional support to patients and families dealing with chronic illness, disability, or end-of-life issues.
- Encourage patients’ independence and involvement in their own care when possible.
- Address mental health and psychosocial concerns, referring to specialists when needed.
