Key Responsibilities:

1. Patient Assessment:
   The Home Care Nurse conducts thorough assessments of patients' physical, emotional, and psychological health upon their first visit and periodically thereafter. This includes taking medical histories, evaluating vital signs, and assessing the patient’s overall condition, mobility, and needs.

2. Developing Care Plans:
   Based on the assessment, the nurse creates individualized care plans that address the specific health needs of the patient. These plans are developed in collaboration with the patient, family members, and the broader healthcare team. The nurse regularly reviews and updates the care plan as necessary to ensure optimal health outcomes.

3. Administering Medications:
