Position Summary:
A Home Care Nurse provides medical care to patients in their homes, ensuring that individuals receive the necessary treatment and support to recover or maintain their health in a comfortable and familiar environment. The role combines clinical expertise with compassionate care to assist patients who may have chronic illnesses, post-surgical recovery needs, disabilities, or age-related health issues. Home Care Nurses assess, plan, implement, and evaluate care needs while working closely with family members and other healthcare professionals to provide holistic, patient-centered care.
Key Responsibilities:

Patient Assessment:
The Home Care Nurse conducts thorough assessments of patients' physical, emotional, and psychological health upon their first visit and periodically thereafter. This includes taking medical histories, evaluating vital signs, and assessing the patient’s overall condition, mobility, and needs. -
Developing Care Plans:
Based on the assessment, the nurse creates individualized care plans that address the specific health needs of the patient. These plans are developed in collaboration with the patient, family members, and the broader healthcare team. The nurse regularly reviews and updates the care plan as necessary to ensure optimal health outcomes. -
Administering Medications:

Wound Care and Dressing Changes:
