Job Description
Key Responsibilities

Patient Care at Home
-
Provide direct care such as administering medications, injections, and treatments as prescribed by a physician.

Monitor and record vital signs including temperature, blood pressure, heart rate, and respiratory rate.

Assist patients with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and feeding.

Help with mobility and transfer, including moving patients from beds to wheelchairs and helping them walk if needed.

Perform wound care and dressing changes for surgical or chronic wounds.

Monitoring Health Conditions

Observe and assess patients for changes in health status and report significant changes to physicians or family members.

Keep accurate, up-to-date records of patient care, medications, and progress.

Manage chronic illnesses such as diabetes, hypertension, COPD, or heart disease by following and adapting care plans.

Medical Support and Equipment Management
-
Operate and maintain medical equipment such as oxygen tanks, feeding tubes, catheters, or ventilators.

Educate patients and caregivers on how to use and maintain any in-home medical devices.

Ensure cleanliness and proper infection control procedures are followed to maintain a safe home environment.

Coordination of Care

Work closely with doctors, therapists, and other healthcare providers to implement and adjust individualized care plans.

Communicate regularly with family members or guardians about the patient’s condition and needs.

Assist with scheduling follow-up appointments, therapy sessions, or additional medical services.

Emotional Support and Companionship
-
Offer companionship and emotional support to patients who may feel isolated or depressed.

Provide comfort and a sense of security, especially for elderly or terminally ill patients.

Encourage and assist patients with hobbies, light exercise, or activities to promote mental well-being.

Health Education

Educate patients and their families on disease prevention, medication management, nutrition, and hygiene.

Teach safe home practices to avoid injuries or accidents.

Support caregivers with guidance and training so they can assist effectively when the nurse is not present.
