Job Description
Medical Coder will be responsible for supporting medical professional corporation procedural and diagnostic coding of medical records for billing. A medical coder should know a wide range of codes, including Current Procedural Terminology (CPT), Healthcare Common Procedure Coding System (HCPCS), and International Classification of Diseases Tenth Revision Clinical Modification (ICD-10 CM).

Knowledge in Anatomy and physiology.
Good computing knowledge in Microsoft word, Excel, PowerPoint etc.

Key Responsibilities:

Reviewing Clinical Documentation:

Read and analyze medical records provided by physicians, nurses, and other healthcare providers.

Understand the diagnoses, procedures, services, and treatments described in the documentation.

Ensure completeness and accuracy of the medical documentation before coding.

Assigning Medical Codes:

Use ICD-10-CM codes for diagnoses and CPT or HCPCS codes for procedures and services.

Apply modifiers and other coding conventions where required.

Ensure codes are selected according to official coding guidelines and payer policies.

Auditing and Quality Control:

Perform regular audits on medical records to ensure accurate coding and billing.

Correct coding errors identified through audits or insurance claim denials.

Communicate with healthcare providers to clarify ambiguous documentation.

Insurance and Reimbursement Support:

Work closely with billing teams to submit accurate insurance claims.

Respond to coding-related queries from insurance companies or government payers.

Assist in resolving claim rejections and denials by providing detailed explanations or corrections.

Compliance and Confidentiality:

Follow all applicable regulations including HIPAA for patient confidentiality.

Adhere to coding standards and ethical guidelines as set by organizations such as AAPC, AHIMA, or CMS.

Stay current on healthcare laws and insurance policies that affect coding.

Reporting and Record Keeping:

Maintain logs and records of coded documents.

Generate coding productivity and accuracy reports for management.

Participate in data analysis and reporting for clinical and administrative use.
