Key Responsibilities
The daily duties of a medical coder include a variety of tasks centered around reviewing, coding, and submitting healthcare data. The main responsibilities are as follows:
1. Reviewing Clinical Documentation

Examine patient records and physician notes to determine the appropriate codes for diagnoses and procedures.

Analyze documentation to ensure it is complete and accurately reflects the patient’s condition and services rendered.

Communicate with healthcare providers when documentation is unclear or incomplete, requesting additional information as needed.
2. Assigning Correct Codes
-
Apply the correct Current Procedural Terminology (CPT), International Classification of Diseases (ICD-10), and Healthcare Common Procedure Coding System (HCPCS) codes to each medical service.

Ensure that codes assigned are in compliance with regulatory guidelines and coding rules.

Use coding software and reference manuals to identify the most accurate and up-to-date codes.
3. Submitting Claims

Collaborate with billing departments to ensure accurate and timely submission of medical claims to insurance companies.

Review claims for completeness and accuracy before submission.

Monitor claims status and follow up on denied or delayed claims.
4. Maintaining Records and Documentation

Maintain organized, up-to-date records of coded patient data for auditing and reporting purposes.

Safeguard patient confidentiality in compliance with HIPAA regulations.

Assist in quality assurance audits and internal reviews.
5. Compliance and Regulation
-
Stay informed about changes in coding standards, insurance requirements, and healthcare laws.

Follow ethical coding guidelines and maintain compliance with national and state healthcare regulations.

Participate in ongoing training and professional development related to medical coding and healthcare compliance.
