Job Summary:
We are looking for a highly skilled Medical Coding Documentation Analyst to join our healthcare team. The Medical Coding Documentation Analyst will be responsible for reviewing clinical documents and accurately coding medical diagnoses, procedures, and treatments as per industry standards and guidelines. This role is critical in ensuring that healthcare services are properly documented and that the coding complies with regulatory and payer requirements.
The ideal candidate will have a strong understanding of medical terminology, coding systems (ICD-10, CPT, HCPCS), and healthcare documentation. Experience in coding and knowledge of healthcare regulations are key components for success in this position.
Key Responsibilities:
1. Medical Coding and Documentation Review

Review and analyze patient medical records to assign appropriate ICD-10, CPT, and HCPCS codes for diagnoses, treatments, and procedures.

Ensure that medical coding is accurate, complete, and compliant with healthcare regulations and payer requirements.

Cross-reference medical records with the clinical documentation to verify completeness and accuracy.

Flag any discrepancies or documentation gaps and work with healthcare providers to improve the quality of documentation.
2. Compliance and Regulation Adherence

Stay updated on current coding standards, regulations, and payer-specific requirements, ensuring the proper implementation in coding practices.

Ensure that all coding complies with HIPAA guidelines and confidentiality policies to protect patient information.

Assist in auditing clinical documentation and coding to ensure adherence to policies, laws, and industry best practices.
3. Query and Clarification

Communicate effectively with healthcare providers, including physicians and nurses, to clarify documentation or obtain additional information as needed.

Submit appropriate queries for ambiguous, incomplete, or conflicting documentation to ensure accurate coding.

Maintain an organized system for tracking queries, responses, and coding adjustments.
4. Coding Accuracy and Quality Assurance

Maintain high levels of accuracy and productivity in the coding process, meeting organizational standards.

Participate in internal and external audits, assisting with reviews of clinical documents and coding accuracy.

Provide feedback and training to healthcare providers and other team members to improve documentation quality.
5. Reporting and Documentation

Prepare and maintain documentation of coded procedures and diagnoses for future reference and audits.

Generate and review reports related to coding activities, providing insights on trends, errors, and performance metrics.

Work with the coding manager to assess and report on key performance indicators (KPIs) like coding accuracy, turnaround time, and query resolution rates.
Required Skills and Competencies:
