Job Description
A Medical Coding Specialist reviews patient records and assigns standardized codes for diagnoses, treatments, and procedures using ICD-10, CPT, and HCPCS systems. They ensure accurate billing and compliance with healthcare regulations.
Job Responsibilities:-
Review and analyze medical records for coding accuracy.

Assign appropriate ICD-10, CPT, and HCPCS codes.

Ensure coding compliance with healthcare guidelines.

Work with healthcare providers to clarify documentation.

Support billing and insurance claim processes.
Key Responsibilities:
1. Medical Record Review and Code Assignment

Review patient records, medical charts, physician notes, lab reports, operative reports, and other clinical documents.

Assign accurate ICD-10-CM codes for diagnoses and CPT/HCPCS codes for procedures and services.

Ensure coding reflects the highest level of specificity and aligns with physician documentation.
2. Insurance and Billing Compliance

Ensure that coding practices comply with HIPAA, CMS guidelines, and payer-specific rules.

Work closely with billing teams to ensure correct data submission and prevent claim denials or rejections.

Help identify and correct coding discrepancies that could result in underpayment or audit risk.
3. Query and Documentation Improvement

Query physicians or healthcare providers for clarification if documentation is insufficient or ambiguous.

Suggest improvements in documentation to support accurate coding and billing.

Educate providers on proper coding practices when necessary.
4. Auditing and Quality Assurance

Participate in internal or external coding audits to ensure adherence to coding standards and accuracy benchmarks.

Analyze audit feedback and take corrective action as needed.

Maintain coding accuracy rate as per organizational standards (typically 95% or higher).
5. Data Entry and Electronic Medical Records

Enter coded data into the Electronic Health Records (EHR) or Medical Billing Software accurately and promptly.

Review and verify the information for completeness before final submission to insurance companies.

Maintain records of coded charts and work completed.
6. Denial Management and Appeals

Investigate denied claims and identify root causes related to coding issues.

Provide corrected codes or documentation for resubmission.

7. Training and Development
