Job Description
Job Summary:
A Medical Coding Specialist is responsible for reviewing clinical documentation and assigning appropriate standardized codes for diagnoses, procedures, medical services, and equipment. These codes are essential for accurate billing, insurance reimbursement, and maintaining legal compliance with healthcare regulations.
The Medical Coding Specialist bridges the gap between healthcare providers and insurance companies by translating complex medical data into universally accepted alphanumeric codes, ensuring healthcare organizations are compensated properly and that patient records are accurate and up to date.
This role requires a strong attention to detail, a deep understanding of medical terminology, anatomy, healthcare procedures, and coding systems such as ICD-10, CPT, and HCPCS. The position may also involve regular collaboration with physicians, billing teams, and auditors.
Key Responsibilities:
1. Medical Record Review:

Analyze and review the patient medical records, clinical notes, lab reports, operative notes, discharge summaries, and other relevant documentation.

Ensure documentation supports diagnoses and procedures being coded.
2. Accurate Code Assignment:

Assign proper ICD-10-CM (International Classification of Diseases, 10th Revision, Clinical Modification) diagnosis codes.

Assign CPT (Current Procedural Terminology) codes for procedures and services performed.

Use HCPCS (Healthcare Common Procedure Coding System) Level II codes for non-physician services, supplies, and medications.

Ensure codes are applied according to official guidelines and payer policies.
3. Compliance and Quality Assurance:

Maintain compliance with federal regulations such as HIPAA, coding guidelines, and payer-specific rules.

Follow ethical coding practices and organizational standards.

Participate in internal audits and reviews to ensure coding accuracy and quality.

Respond to feedback or queries from auditors and revise codes as necessary.
4. Denials and Appeals Support:

Assist billing and accounts teams in resolving claim denials related to incorrect or incomplete coding.

Provide coding clarifications and documentation to support resubmissions or appeals.

Communicate with insurance companies or payers if needed, to clarify coding and support reimbursement.
5. Collaboration & Communication:

Work closely with physicians, nurses, and healthcare staff to obtain accurate and complete clinical information when needed.

Provide feedback to medical staff on documentation quality and improvement areas.

Communicate with billing or revenue cycle teams to ensure seamless workflow.
6. Data Entry & Record Keeping:

Accurately input coded data into electronic health record (EHR) systems or medical billing software.

Maintain records of coded data for reporting, auditing, and reference.

Monitor coding queues and manage workload to meet deadlines and productivity targets.
7. Continuous Learning:

Stay updated with changes in medical coding systems (ICD, CPT, HCPCS), payer policies, and industry best practices.
