Responsibilities include the following and other duties may be assigned
Explain the features of the Relex Medication Therapy Management Program to prospective or existing customers/patients
Effect medication replenishment, dosage tracking and obtain patient feedback.
Monitor outcomes and follow up on reminders to patient against counselling notes from Clinical Pharmacist and required lab tests
First level patient counselling and guidance on correct use of medication for achieving desired/targeted outcomes
Communicate customer feedback and customer experience on products/services and/or modifications to existing products/services to Operations, Marketing and Quality teams.
Must be able to work both with external and internal customers.
Comfortable in new environments and adverse conditions. Must be able to work and interact with diverse groups of people.
Provide information and explanation of diagnoses, options, outcome and risk and escalate where required
Data updation in all tracking platforms on a daily basis
Day to Day responsibilities
Follow up and adherence to customer visit schedules against preset appointments
Monitoring of key vitals and ADRs and recording of progress.
Replenishment of prescription medicines and tracking of consumption against prescribed dosage.
Follow up on patient action against reminders and lab tests

Requires morning and evening flexibility as necessary to meet customers’ schedules and coordinate accordingly
