Job Description
Review and dispense prescriptions accurately and efficiently.

Provide counseling to patients on proper medication usage, dosage, side effects, and interactions.

Ensure compliance with all applicable laws and regulations related to the practice of pharmacy.

Maintain accurate patient medication records and documentation.

Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.

Manage inventory, including ordering, receiving, and storage of pharmaceutical supplies.

Monitor for drug incompatibilities, therapeutic duplications, and potential allergies.

Stay updated on new medications, treatments, and pharmaceutical regulations.

Participate in continuous quality improvement and safety initiatives.
Key Responsibilities:

Medication Management and Dispensing:
-
Accurately dispense prescribed medications to patients in accordance with healthcare professionals’ instructions.

Ensure correct dosages, medication forms (e.g., tablets, injectables), and administration methods are followed.

Review prescriptions for accuracy, legality, and appropriateness, and resolve any discrepancies with the prescribing doctor.

Maintain proper storage and inventory management of medications, ensuring compliance with hospital policies and safety regulations.

Patient Counseling and Education:

Educate patients on proper medication use, potential side effects, and drug interactions.
-
Provide advice on the administration of medications (e.g., timing, food interactions, and dosage instructions).

Counsel patients on over-the-counter (OTC) medications and the safe use of herbal supplements and alternative therapies.

Address patients’ concerns about their medications, including explaining why a specific medication is prescribed and the importance of adherence to the prescribed regimen.

Collaborating with Healthcare Team:

Work closely with doctors, nurses, and other healthcare professionals to ensure coordinated and safe medication therapy management.

Provide clinical advice to the healthcare team on drug choices, dosage adjustments, and potential drug interactions based on patient-specific factors.

Participate in patient care rounds and contribute to multidisciplinary team discussions regarding medication therapy plans.

Review and monitor ongoing treatments and adjust medication regimens as necessary in response to patient conditions.

Clinical Monitoring and Drug Therapy Management:

Monitor patients for adverse reactions, therapeutic effectiveness, and medication interactions, and adjust treatment plans accordingly.

Review patient medical records and lab results to assess the appropriateness of prescribed medications.

Ensure that medications are administered safely and provide necessary interventions for high-risk or complex cases (e.g., chemotherapy, anticoagulant therapy).

Participate in therapeutic drug monitoring (TDM) for high-risk medications such as antibiotics, anticoagulants, and other critical drugs.

Pharmacy Operations and Compliance:

Maintain accurate patient medication records, ensuring confidentiality and compliance with hospital and regulatory standards (e.g., HIPAA, FDA guidelines).

Ensure all medications and pharmaceutical supplies are handled, stored, and disposed of according to hospital policies and legal requirements.

Monitor and manage inventory of medications, placing orders for drugs when needed, and ensuring adequate stock levels.

Oversee the preparation and sterilization of intravenous (IV) medications and ensure the accuracy of drug preparation.

Keep up-to-date with drug formulations, new medications, and medical guidelines to ensure that hospital practices are aligned with current standards.

Patient Safety and Regulatory Compliance:

Adhere to the highest standards of patient safety and ensure the safe distribution of medications to avoid errors.

Report medication errors, adverse drug reactions (ADRs), and side effects through the appropriate channels.

Educate hospital staff on safe medication practices, and assist with training on drug-related topics.

Maintain knowledge of national and international pharmaceutical regulations, such as FDA guidelines, to ensure hospital compliance.

Pharmacy Staff Supervision and Mentorship:

Provide guidance and support to pharmacy technicians and interns in the daily operations of the pharmacy.

Ensure that all pharmacy staff are adhering to hospital policies and regulations.

Participate in the training and continuing education of new pharmacy staff, interns, and other healthcare professionals.
