Job Description
Review and dispense prescriptions accurately and efficiently.
Provide counseling to patients on proper medication usage, dosage, side effects, and interactions.
Ensure compliance with all applicable laws and regulations related to the practice of pharmacy.
Maintain accurate patient medication records and documentation.
Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.
Manage inventory, including ordering, receiving, and storage of pharmaceutical supplies.
Monitor for drug incompatibilities, therapeutic duplications, and potential allergies.
Stay updated on new medications, treatments, and pharmaceutical regulations.
Participate in continuous quality improvement and safety initiatives.
Key Responsibilities:
1. Medication Dispensation & Management:

Prepare and dispense prescription medications to patients, ensuring accuracy and adherence to the prescribed dosage and form.

Verify prescriptions for accuracy, dosage, and potential drug interactions.

Ensure that the proper storage of medications is maintained, especially for controlled substances and temperature-sensitive drugs.

Monitor the stock levels of drugs and reorder medications as required to avoid shortages.
2. Patient Safety & Counseling:

Provide counseling and guidance to patients regarding the correct use of medications, including side effects, dosage instructions, and potential drug interactions.

Educate patients on lifestyle changes or any other complementary therapies they may need in conjunction with their medication.

Work with the healthcare team to review and assess patients' medical histories and medication regimens to ensure safety and efficacy.
3. Clinical Support:

Collaborate with doctors and nurses in the development and adjustment of patient medication regimens, providing input on potential alternatives or drug interactions.

Participate in multidisciplinary rounds, offering advice on drug therapy and monitoring patient progress.

Monitor and assess the effects of medications on patients and adjust therapy as necessary in response to clinical feedback.
4. Drug Information & Research:

Provide accurate, evidence-based drug information to healthcare providers to aid in decision-making.

Stay up-to-date with the latest pharmacological research, drug approvals, and medical advancements to improve patient care.

Participate in research projects to evaluate the effectiveness and safety of drugs in hospital settings.
5. Inventory Management & Procurement:

Maintain an accurate inventory of medications and supplies, ensuring the stock is regularly updated, and expired drugs are removed from circulation.

Order new medications and supplies based on the hospital's needs, ensuring the quality of the products is up to standard.

Maintain records and documentation related to medication inventory, purchases, and usage.
6. Regulatory Compliance & Documentation:

Ensure that the pharmacy operates in compliance with hospital policies, local regulations, and legal requirements related to drug dispensation and pharmacy practices.

Keep detailed records of medication prescriptions, patient treatments, and stock management.

Follow proper protocols when handling and disposing of hazardous medications or controlled substances.
7. Education & Training:

Provide training and guidance to junior pharmacy staff and pharmacy students on safe and effective pharmaceutical practices.

Participate in patient education programs within the hospital regarding medication management and disease prevention.

Keep up with continuing professional development to stay current with industry standards and pharmaceutical practices.
8. Emergency Support:

Respond to and manage medication-related emergencies, such as drug overdoses or adverse reactions.

Work as part of the emergency department team during critical situations, assisting in providing appropriate medications and formulations for patients.
