We are seeking a dedicated and knowledgeable Registered Pharmacist to join our innovative healthcare team. Responsibilities:
Manage and oversee the medication therapy management process, ensuring optimal patient outcomes through personalized medication plans.
Conduct thorough medication reviews and provide comprehensive consultations to patients and healthcare providers regarding medication therapies.
Monitor patient medication regimens, identifying potential drug interactions, allergies, or contraindications and providing actionable recommendations.
Educate patients on proper medication usage, side effects, and adherence strategies to promote optimal health outcomes.
Collaborate with multidisciplinary healthcare teams to develop integrated care plans and enhance patient care.
Stay up-to-date with advancements in pharmaceutical sciences, regulations, and best practices to ensure compliance and quality care.
Qualifications:
Valid Registered Pharmacist license in the applicable state.
Doctor of Pharmacy (PharmD) degree from an accredited institution.
Strong knowledge of pharmacology, therapeutics, and medication management.
Exceptional communication skills, both verbal and written, with the ability to convey complex information clearly.
Proficiency in electronic health record (EHR) systems and pharmacy management software.
