Job Description
Key Responsibilities:
1. Patient Assessment and Care Planning:
-
Conduct comprehensive initial assessments to understand the patient’s condition, care needs, and medical history.

Develop individualized care plans in coordination with the physician, family, and home healthcare team.

Reassess the patient’s health status regularly and revise care plans as needed.
2. Nursing Care & Clinical Duties:
-
Administer medications, IV fluids, and injections as per doctor’s prescription.
-
Monitor and record vital signs including temperature, blood pressure, heart rate, and respiration.
-
Perform wound care, catheterization, suctioning, tracheostomy care, and other clinical procedures as required.

Manage chronic conditions like diabetes, hypertension, or COPD under medical guidance.
3. Documentation & Reporting:
-
Maintain accurate records of all nursing interventions, medications administered, and patient responses.

Document daily observations and activities in patient care reports and submit them to the assigned supervisor or physician.

Notify the doctor or clinical coordinator of any abnormal symptoms or complications.
4. Patient & Family Education:
-
Educate patients and caregivers on medication routines, post-operative care, hygiene, nutrition, and rehabilitation exercises.

Provide training to family members for basic care tasks when applicable.

Offer emotional and psychological support to patients and their families.
5. Emergency Handling:
-
Identify early signs of medical emergencies and take immediate action.

Perform CPR and other life-saving interventions in case of critical situations until emergency services arrive or the patient is stabilized.

Ensure emergency protocols are followed and that family members are guided calmly through crises.
6. Coordination & Communication:
-
Communicate regularly with physicians, physiotherapists, and other healthcare professionals involved in the patient’s care.

Liaise with family members and ensure they are kept informed of the patient’s condition and progress.

Maintain patient confidentiality and ensure ethical care standards are upheld.
7. Mobility Assistance & Rehabilitation Support:
-
Assist patients with basic movements, daily exercises, walking, and rehabilitation activities as prescribed.

Provide bed care to bedridden patients, including hygiene, turning, and feeding assistance.

Prevent bedsores through proper positioning and skin care techniques.
8. Infection Control & Hygiene:
-
Ensure high standards of hygiene and sanitation in the patient’s environment.

Use proper PPE and follow infection control protocols during patient care activities.
-
Educate patients and caregivers on safe handling of medical equipment and materials.
