Job Description
- Gather pertinent information about the patient's medical and family history-record information in the forms provided.
- Coordinate with the counselors and patients to assist in organizing appointments while closely working with the patient caregivers and doctor's/hospital's team.
- Assist cancer patients/caregivers with accessing all the available resources for cancer patients that are provided by the doctor/hospital.
- Work with the central team to provide counseling support and community program support to patients based on their need.
- Provide emotional support to the patients as needed.
- Be an advocate for the patient.
