The Medical Social Worker is responsible for providing psychosocial support, counseling, and guidance to patients and their families. The role focuses on helping patients overcome social, emotional, financial, and environmental challenges related to illness, hospitalization, treatment, and discharge planning. Key Responsibilities:
- Assess patients' social, emotional, and financial needs.
- Provide counseling and emotional support to patients and their family members.
- Assist patients in availing government health schemes and welfare benefits.
- Coordinate with doctors, nurses, and other healthcare professionals for patient care planning.
- Facilitate discharge planning and post-hospital follow-up services.
- Refer patients to NGOs, rehabilitation centers, community services, and support groups.
- Support economically weaker patients in obtaining financial assistance.
- Handle crisis situations, grief counseling, and family support.
- Maintain patient case records, reports, and documentation.
- Conduct patient education and health awareness activities.

Qualification:
- Master of Social Work (MSW) from a recognized university.

Experience:
